Our spray tanning solution does not contain a sunscreen and does not protect against sunburn. (Title 21 of the Code of Federal Regulations, Section 740.19 USA).
I'm aware that freckles and sun spots may temporarily darken with spray tans. I am aware that waxing, retinols, acne and exfoliating skin treatments may cause unevenness or early fading with my spray tan.
While there are no known contraindications, if you are an expectant or a lactating mother you acknowledge that you accept all responsibility towards any contraindications that may occur.
Cosmetic bronzers are water soluble and will wash out of most fabrics. Please note that you may experience staining on various surfaces or fabrics. We recommend loose, dark clothing be worn after the completion of your spray tan.
Some bleached /dyed or treated hair types, if uncovered, may stain or change from their original color. This can also apply to eyebrows or other body hair. I understand hair nets are provided to me free of change.
I understand that eye goggles, nasal plugs and lip balm are recommend by the FDA and provided at no cost by Mint Tan. I will request these items if I choose to wear them.
I understand that it is not recommended to spray tan within 6 weeks of a sunburn. Peeling will be significantly exaggerated by the difference in skin color, where dark tanned skin and fresh new skin will have a severe demarcation.
I, the undersigned, agree that I fully understand this Spray Tanning Waiver. I am using these services at my own risk. I hereby authorize and direct employees or agents to perform such tanning procedures as may be deemed necessary or advisable, and have provided them with the above information required.
I acknowledge that the results of spray tanning do vary, and no guarantees of specific results are offered or implied. I agree to follow all of the skin preparation and the maintenance care instructions to ensure the best possible results. I hereby relieve Mint Tan LLC, its agents, employees and contractors, and hold them unliable, from any liability involved in the use of spray tanning.
